Forms

Permission forms are needed for youth to attend all events where transportation is required or youth are staying overnight. A new permission form is required for each event. Medical release forms are valid for one school calendar year.

Medical Release Form 2011-2012

The Medical Release Form only needs to be filled out once a year and can be used beginning August 2011 until the end of July 2012. Parents are responsible for communicating any policy changes throughout the year.
